Help Us, Help You!

When you have a medical emergency, it is important that responders know about pre-existing medical conditions and current medications. Having this information ensures you receive the best possible care in the shortest time.

Complete an AFD Emergency Medical Card and post it on your refrigerator.  This card provides our responders with critical information such as current medications, your physician's name, past surgeries, and emergency contacts.  You may also include the location of advanced directives, including Do Not Resuscitate (DNR) orders.

Also available is this Personal Medication Record from AARP.
